If your Assistance Dogs UK identification card, certificate or certification letter has been lost, stolen, damaged or has become unreadable, you can request a replacement using the form below. Please provide your registration details, confirm which items need replacing and explain whether any information has changed. Once your registration has been verified, updated digital documents will be issued and any requested physical replacements will be prepared for delivery. Replacement fees apply.
