Register Your Assistance Dog
Use the registration form below to register your assistance dog with Assistance Dogs UK. Please provide accurate information about yourself, your dog, your disability-related needs and the assistance your dog provides. You may also be asked to submit supporting medical documentation confirming that you have a qualifying visible or hidden disability and require the dog as an auxiliary or medical support aid. Once submitted, your application and supporting information will be reviewed before registration documents and identification are issued

Mini Registration — £39.00
The Mini Registration package includes:
- Assistance Dogs UK identification card — physical and digital
